  • "How do I get the Special Permission number - - - Credits?" - see your academic department. "Tuition Deferment: IT IS NOT AUTOMATIC! - Do I need one?" - see the Business office section. "Housing Questions and requests?" - see the Housing office section. "Financial Aid: Scholarships, Loans, Grants - - Questions of all sorts?" - see the Financial Aid office section. "Where do I pay my tuition?" - see the Business office section.

  1. Student Cooperative Education forms needed for registration and departmental approval:

The Student Agreement and Release Form requires student and Cooperative Education office signatures and must be on file at the Co-op office. When signed students give permission for their information to be filed (both electronically and/or manually) and released to prospective employers.

The Department Recommendation form is required for all students. It is the document that assures students they they meet the department requisites to receive co-op credit. Students do not need to meet the total requirements of this form when they are seeking co-op opportunities. However, before the can receive co-op credit for a co-op work term, this form must be signed by the department co-op coordinator.



  2. Student Cooperative Education forms required after you have agreed to a co-op position:

The Agreement form is require for all students working in a co-op position and receiving co-op credits. A new form must be originated for each co-op work term. It must be signed by the student, the students supervisor and returned to the co-op office for the remaining signatures. A copy will be in the students co-op file.

The Student Employment Data form is required for all students working in a co-op position and receiving co-op credits! A new form must be originated for each co-op work term.


The Student Job Objective form is required for all students working in a co-op position and receiving co-op credits; Except - Accounting and Aviation students! A new form must be originated for each co-op work term.


The Student Mid-Term Report is required for all students working in a co-op position and receiving co-op credits; Except - Accounting and Aviation students! A new form must be originated for each co-op work term.


The Student Final Report is required for all students working in a co-op position and receiving co-op credits; Except - Accounting and Aviation students! A new form must be originated for each co-op work term.


The Student Exit Evaluation Report is required for all students working in a co-op position and receiving co-op credits. A new form must be originated for each co-op work term

a. Forms required to be completed by your employer/supervisor:

Student's must give their supervisor a copy of this Employer's Mid-term Evaulation form at the mid-term of the semester for the supervisor to complete and return to the Co-op Office. A new form must be originated for each co-op work term.


Student's must give their supervisor a copy of this Employer's Final Evaulation form at the mid-term of the semester for the supervisor to complete and return to the Co-op Office. A new form must be originated for each co-op work term.

b. Forms required by Mechanical and Chemical Engineering Student's:

Mechanical & Chemical Engineering student's must complete these forms and return them directly to the Mechanical Engineering or Chemical Engineering office. A new form must be originated for each co-op work term.
